Unhappy electrical prob?

I have a 91 crx hf (1.5 sohc 8 valve) when i am running the car voltage is fine for a while. then as i hit the throttle voltage goes down. replaced alternator, still the same but if i unplug voltage regulator voltage stays at 13.88 but then i have only 1/4 throttle and if i push it to the floor nothing happens. Also if i am driving car will not let me go past 4,000 rpms until warmed up but lose power after 4,100. wondering if it is a wiring problem?
