at that point you would be better off saving your money and getting a B16 or GSR head and working with that

the B18C/R is a great engine, in stock form... with a few bolt ons it can be made even better...
but it is not a "good starting point"... if you are going to be ripping it apart and making all sorts of changes to the internals, then starting with an ITR swap is a waste of money...

No, you don't necessarily NEED it. Keep in mind that you'll have an automatic gear indicator display that's permanently lighted up in "P" though. That would bother me, but if you're cool with it then no need to upgrade...

Any D-series tranny will work, yes. I'd look for a D16Z6 transmission though. It will have the best gearing. the mounting bolts on a z6 head will not allow you to set timing at 16*btdc unless you use a td42u distirbutor.
and a p28 is obd1 (hence the obd1 conversion reference in my previous post)
