Besides como esta usted? is how are you in the proper correct formal way that no one uses.

My take is most people who know each other don't want to say como esta "anything" because its too formal. Como Esta! is enough by itself unless you're just meeting someone

For normal talks I say eh-lo "hello" or que onda/que tal. Sometimes que pasa

If I don't know the person or they are older I'll say como esta usted or ustedes. if younger, I say tu. but, only if I don't know them.
