My engine temperature seems to be higher than it should be and every time I drive constantly for over 15 minutes, my radiator fan kicks in by itself. Does this more than likely mean that I need a coolant flush or is it something else. I drive a 92 Prelude.

if the coolant is more than 2 years old you should flush it any way
if that doesnt work check the thermostat
 
You should flush your coolant once a year. Also every % years you should change your radiator cap, because the seal gets old and worn out. this will cause leaks around the seal and you will loose some pressure on your radiator. In which the result is higher engine temperatures! I say flush it, change the cap(if older than 5 years) then if that don't fix it... Look into the thermostat!
