Depends on wheel size and tire size... lots of people stick to +42 with a 7 inch wheel and a 205 tire, assuming that the tire diameter is sized correctly for your transmission.
 
proper offset is determined by a couple things.

- wheel width
- brake caliper/etc clearence needed
- fender design
- tire height (to clear above)

the bolt pattern say 4x100mm, 5x100mm is how far apart the studs are.
offset is the distance from the centerline of the rim to the hub
