tranny or clutch?

i've got a 91 hatch with a vetec b16.... i cannot put into any gear while the car is started...and if i turn the car off press in the clutch and put into 1st, when i turn the car over it jumps like i don't have the clutch in and dies? clutch or tranny or what do i need to fix here?

clutch problem - try adjusting it

how does one go about adjusting the clutch?
