2 weeks notice

i am getting ready to move back to Virginia soon and unfortunately i am leaving a nice job here in Indiana. i need to type up a 2 weeks notice. does anyone know how to do one and what to say? sorry for the dumb question, but i am kinda lost. thanks for the help all.
