You are missing the bracket from the DA. It's common for that not to come with the swap. I had to find one for my swap too.
I have not heard anything about using only two urethane mounts, but it seems like having a third would be better.
I passed the referee check last year with a '91 B18 in my 88 CRX.

Take the product number from the side of the distributor flange that starts with TD
Then compare that number to the list HERE and you will know which car it is from and the OBD.
Is this the dust shield on the back of the spindles? I don't remember removing anything when I swapped mine. Why must this part be removed? Thanks for the info
