I advise to not put it in yourself, all the resistors and small bits are easily wrecked, and unless you know alot about electronics, i suggest taking it to a place that knows what they are doing and will do it for like 50 bucks. might cost more but you know its not fucked.
 
You'll have to de-solder the eeprom from the circuit board. Unless you are very good with a soldering iron I suggest you don't do it yourself.
