If your fuel system has any kind of high miles on it I would get the fuel rail and injectors cleaned. Having dirty injectors restricts fuel flow and more importantly can cause the spray pattern to be off and not atomize the fuel correctly. If the problem is worse when its cold and gets better and higher rpms/engine temp than I would be thinking of a fuel issue for sure. From what I have seen your best bet for cleaning your injectors is removing them and having them ultrasonically cleaned or to have a motor-vac service performed by a shop.
