12mm grade 8 14mm coarse bolt with large flat washer.
Cut the end of the strut open. You do not need to cut through the strut, but around it just under the welded area. Degrease the tube and deburr the cut edge.
This is what comes out.
Turn the housing over, and drill a 9/16ths hole in the bottom on center.
When everything is all cleaned up, stick the insert into your fresh strut housing.
Put the bolt through the bottom. It will pull the insert down a little. I torqued down to 55nm as stated by Koni and measured left and right to be the same length as each other, but the overall assembly was 1/2 inch higher than stock.
